Would appear that posts do not get displayed due to a cache rebuilding process that does not seem to fire automatically.

one more test. The Mod user post was posted immediately. The posts from this account are still waiting in the approval queue, and are not displayed to the users without mod status.

Doesn't seem to be anything wrong with the quick reply form, might just have been a stale cache issue. If you managed to post just on the cusp of the last cron run, it would take about 5 minutes for the page to be cleared from cache and rebuilt.
